"What is clear from Vice Admiral Ritchie's report is that the risk of a casualty occurring in Afghanistan was increased by the deficiencies in training, assessment, certification and leadership."

This is a huge concern - it appears as though the problem is systematic rather than isolated. How many other front-line commanders are comprimising Australian soldiers lives by being deficient in the areas of training, assessment, certification and leadership ?

The commanding officer involved was relieved of his command but not demoted and the enquiry has concluded that it's own training should be reviewed.
This is obviously difficult to discuss for those that refuse to be critical in any way of our defence forces.
