    The pilot killed in a New Smyrna Beach plane crash earlier this month was not "signed off" for a solo flight, crash investigators said in a new report. Mihoko Tabata, 38, a Japanese citizen, somehow obtained the keys for the plane and was flying it without the knowledge of owner, according to a preliminary report from the National Transportation Safety Board.
