On May 28, 2015 Roy Mullins, 45 years old of Mouthcard, Kentucky, was attempting to attach a chain between a tractor trailer and grader when the accident occurred.

    Police say Roy Mullins, 45, of Mouthcard was attempting to attach a chain between a tractor trailer and grader when the accident occurred. An investigation at the scene determined that, while working between the two pieces of machinery, the grader rolled back into the tractor, striking Mullins, who then became pinned between the heavy equipment.
