Human error blamed for melted steel accident that injured 13

One of 13 workers who were burned in an explosion at a steel factory in the southern province of Ba Ria - Vung Tau on April 14 Police said human error caused a crucible containing some 150 tons of molten steel to spill, setting off an explosion at a factory in the southern province of Ba Ria - Vung Tau last Friday.
