Nope, I don't have FIOS for my TV. I finally bit the bullet and moved from Comcast to DirecTV so that I could get NFL Season Ticket. I was missing my Chicago Bears too much (sorry Eagles fans). So far I love it. Only had two very short outages during bad storms in the last 5 months. Far fewer than I used to have with Comcast. As I mentioned before the ONLY thing I miss about Comcast is the "On Demand" feature. But the DVR makes up for it for the most part. BTW, what's QVC? ;-) (Nov 14, 2007 | post #9)
Wow Lights that's great! You must have the second tier service. I've got the third tier FIOS which gets me 5200 down and 1900 up. Not bad. If it was much faster I probably wouldn't notice. I've had FIOS for over a year and I have NEVER had a connection or speed problem. I've NEVER had to call them for any problems at all. Had Comcast before and will never use them again (although I do miss the 'On Demand' feature for TV :-). (Nov 13, 2007 | post #6)

FYI - saw that they put up DSW (Designer Shoe Warehouse) and Staples signs next to the Wegman's sign at the new 422 & 29 huge mall entrance. Does it seem strange to anyone else that there would be two Staples so close together? Maybe they're just doing that so Office Max can't move in ;-) AAA, thanks for all the great info. Yep, I've got FIOS and I'm in Trappe. It's about $10 cheaper/month than what I was paying for Comcast. Installation was free and takes about 1/2 a day to complete. I've had it for about a year for my internet service and it has NEVER gone down. I don't have to share a line for bandwidth like you do for DSL so the quick uploads and downloads are consistently fast. FIOS for TV is not avaiable in our area yet so I can't commment on that. I peeked in the window about 2 weeks ago and it was COMPLETELY empty. The walls weren’t even up yet, just metal studs. But they can put up those places pretty quickly so it might not be too much longer. (Aug 9, 2007 | post #20)

I've haven't received the particular form you mentioned but I have gotten several forms in error from Berk. I suggest calling them. In every instance they old me that I "didn't need to fill out that form". Looks like they send these out en masse without much specificity to the particular township.
