It seems that the screw-up was a failure to transmit information from the executive in Texas to the Federal government; perhaps an attempt to avoid those troublesome federal regualations that fill pages with silly stuff like pesky safety proceedures.

The info that the fertilizer company had 500 times too much explosive on site died in thr Texas government.

That failure was in the office of the Governor of Texas, Hon. Rick Perry.

I didn't know that Rick Perry was "... MUSLIM LOVING..."
I only thought he was dumb, incompetant, and full of hot air.
