AS AFTERNOON TURNS to evening in MA ximo GA3mez Park, named for a hero of the independence war against Spain, some three dozen mainly elderly men and a scattering of women play dominoes, banging the tiles down on the ceramic table tops.

Being Cuban AMERICAN, I would not be any stretch of the imagination call the majority of Cubans here American.

Many of us call them Miami Cubans and lower political standards ,I have never seen in USA.

This is not made up by me and a direct reflection on the Miami Cuban majority who run this place.

This is not my idea of an American and this is what I myself have experienced from most Cubans I have met here, Certainly not all but the majority.

"The Cuban community that doesn’t care about corruption when it casts its vote was critically portrayed by most leading city columnists and others, Cuban and otherwise. Robert Steinback, a Black Miami Herald columnist, said the Cuban community is gullible. Liz Balmaseda calls it “a knee-jerk majority” that would elect a hamster wearing a Cuba Libre T-shirt, and,“They only cared that he was Cuban.”

Hermano since republican Jeb Bush signed that law that you can KILL someone if you think that you are in danger, I worry for my safety.

I worry that some crazed gun totting loco como peste will gun me down.

I was going to ask if you're cousin Marco Rubio could do something but than I remembered that he is 100% OWNED by the NRA and works for them not the people.

I might be moving in with you in PR, is this a problem?

Deterioration of mental status is one of the indications for the initiation of hemodialysis.

Peritoneal dialysis is significantly less effective than hemodialysis.

Charcoal hemoperfusion is more effective than hemodialysis, but does not correct acid/base imbalances seen with salicylate poisoning.

Acetazolamide therapy is contraindicated in salicylate intoxication because this drug will also increase the pH of the CSF, worsening the toxic effects of salicylates.

In the patient with salicylate poisoning, some of the indications for hemodialysis include:
acute ingestion with serum salicylate levels greater 100 mg/dL and the presence of acidosis.

chronic toxicity with a blood level > 60 mg/dL accompanied with acidosis, lethargy, renal failure, or in the elderly.

intractable acidosis or electrolyte derangement.

Other indications for hemodialysis include pulmonary edema, renal failure, congestive heart failure, deterioration despite standard therapy, and altered mental status. With salicylate toxicity, treat the patient not the blood level. Tissue levels (which are responsible for toxicity) correlate well with blood levels with acute ingestion, but not with chronic ingestion. With chronic exposure, the patient may have relatively low serum salicylate levels, but high levels of salicylate in the tissues and signs of serious salicylate toxicity. With acute overdose there may be high blood levels, but low tissue levels and no toxicity early in the course. Considerable time is required for equilibration between compartments, so never discharge a patient with a high blood level simply because she or he is currently asymptomatic.

Adults with acute salicylate toxicity most often present with alkalosis from hyperventilation, whereas children are more often acidotic at the time of presentation. CNS and cardiovascular dysfunction, hyperthermia, and coagulopathy develop as acidemia progresses. Salicylates is the “S” in the common mnemonic for wide gap metabolic acidosis, MUDPILES. The acidosis is multifactorial, with salicylic acid, lactic acid and ketoacids contributing to the overall state. Due to tachypnea, the presenting gas often shows a respiratory alkalosis and metabolic acidosis, this is the characteristic lab result for a salicylate overdose./////////

Salicylates are widely available in both prescription and over-the-counter (OTC) preparations. All of the following are true about acute salicylate toxicity .

irritability, lethargy, tinnitus, hallucinations, coma, convulsions and cerebral edema; CSF glucose may be quite low.

metabolic acidosis; blood glucose may be high or low.

hyperventilation and respiratory alkalosis (due to direct CNS stimulation; respiratory depression occurs with very high doses).
