Cancer Drug "Holiday" May Extend Patient Survival

There are 3 comments on the MediLexicon story from Jan 10, 2013, titled Cancer Drug "Holiday" May Extend Patient Survival. In it, MediLexicon reports that:

New research on mice shows that drug-resistant melanoma tumors shrink when treatment is interrupted, or given a "holiday", suggesting that altering the dose pattern of cancer drug treatment in this manner could be a simple way to extend survival in human patients with late-stage disease.

Even in previously failed therapies, results from phenotype analysis have seen numerous occasions in which a drug used earlier becomes effective again, once the tumor has been removed from the selective pressure of drug exposure. The tumor loses the capacity to resist.

Changes induced by subsequent treatment may bring the tumor back to a point where failed drugs are viable again. A number of laboratory oncologists have tested many small molecules and in the past studied the P-glycoprotein and GST inhibitors to reverse drug resistance.

Zelboraf (vemurafenib) exploits the fact that BRAF proteins in healthy cells pair up with other BRAF proteins to form a multiprotein complex, while the mutated BRAF protein acts as a lone compound. This solitary structure can be hundreds of times more effective in activating cell division than the normal paired BRAF complexes.

Zelboraf (vemurafenib) targets tumor cells by only inhibiting the stand alone mutant version, while allowing the twinned version in healthy cells to act unimpaired. Secondary pathways, like NRAS and CRAF, may step to the fore and overtake the effect of the BRAF inhibition.

Using MEK/ERK inhibitors (vemurafenib) and PI3K/mTOR inhibitors, these laboratory oncologists can explore the activities, synergies and possible clinical utilities of these novel compounds, alone and in combination in human tumor primary culture microclusters. Exploration of horizontal pathway targeting.
