If you are on worker's compensation due to a work injury what happens next?

I have been on workers compensation for a while due to a back injury. My doctor has recently put me on permanent restrictions and the type of job I have this will not allow me to go back to work. What happens next? Do I need to get a lawyer or will workers compensation handle it. Where do I go from here?
