This is all too common with the VA system. When I worked there all it took was a phone call from anyone to tell the VA that the veteran was dead. Then all it took was a few strokes in the computer the the veteran is 'claimed' deceased. More training needs to be done in this area and verification needs to be checked.
All it takes is a disgruntle x-wife or friend or hated neighbor to call the VA to give this information and the keystrokes begin. The personnel who input these keystrokes DO NOT verify any information. It is considered an 'outside death' which is USUALLY reported by the local police but anyone can call this information in and the results are absolutely horrible.
Obviously the VA continues in this practice and NOTHING is being done to correct this.
When this happens to you, what will you do?
