going into nursing home..what to expect..

First off, I am only 27 and I am very nervous about this. My doctor suggests that I go this route for inpatient care and rehab.
I am have dealt with multiple health issues since I was a kid and now its lie things are combining and wearing me down. I am dealing with arthritis (R.A.) in my knees and shoulders that seems to be getting worse. I havent been moving around as much as I did just a year ago, my weight is an issue as well.
I had surgery twice for a rectal prolapse and then another surgery to fix damaged muscles but never restored full continence. So im in this situation where I need someone with me 24 hours a day. Even if I do make it to the bathroom in time, I need someone to clean me up because I can't even reach that area because of my shoulders.
Anyways, I will be doing therapy. I have no clue how long I will be there. I am just wondering what to expect. Will I be looked at weird there because of my age ? How do things work ? Is there anything I need to worry about ?
I am determined to get my life and independence back. It is scary with my health declining at 27 and wondering what I feel like if im 60 or if I will make it that long. So I think I need to do this..
