Thank you for the continuing coverage of this terrible tragedy. I am pleased to hear the hospital leadershsip will make every effort to prevent this type of error from occurring again. They must focus on the systems that allowed this error to occur, and other hospitals across the country need to look at that level as well. Making nurses and pharmacists promise to be more careful is not responsive to the root of the problem- confusing medications that even dedicated personnel can mistake during the delivery of patient care. Thank you for the continuing coverage of this terrible tragedy. I am pleased to hear the hospital leadershsip will make every effort to prevent this type of error from occurring again. They must focus on the systems that allowed this error to occur, and other hospitals across the country need to look at that level as well. Before Fresenius' Granuflo and Naturalyte were recalled by the FDA in June, they caused 900 patients to suffer sudden cardiac death. This is likely to cause many, many Granuflo lawsuits. See more info here:

To be hopeful amidst the daily challenges of a serious physical ailment is indeed an admirable trait. Kidney failure is a serious health condition that could even be potentially fatal to those who have it. Although kidney transplant is in place for this health problem, shortage issues in donor kidneys leave most patients relying on dialysis treatment for relief, or even survival. And while benefits of dialysis have been irrefutable, certain formulations used in such procedure, have been linked to adverse, even potentially fatal complications.
