underwear question

OK, it may be a silly question, however I'm compelled to ask.

I'm a 22-year old EKG tech. and phlebotomist (everyone draws blood but they call me first if I'm free) at a medical group office. I've been here for 9 months.

Because 4 of our 6 exam rooms are equipped for EKGs, the patient will often remain in the exam room and await the tech (me), if the doctor has directed an EKG. If the patient is male, I'll often find him clad in only the underpants he was wearing during the exam with the doctor. Many men - not all, but many - eschew the pale blue paper gowns we offer. When this happens, I tell him he can put on his trousers, since it's an EKG. I offer to wait in the hall for modesty purposes. I'm shocked how many men politely refuse the offer and cite "getting it over as quickly as possible" and since they're already prone, they're fine with remaining in their underpants. I'm not a prude. Hardly. But I'm continually astonished at how casual the male patients are while attractive, clothed women milling about, while they're in their unmentionables. Two days ago, a 30 year old stood and talked with me for several minutes after I had finished, and told me about the car he's restoring as if we we neighbors that just crossed paths in the frozen food isle at the local supermarket - all the while wearing nothing but Hanes cotton briefs, sweat socks and sneakers

On rare occasion I've been around naked male patients. When the guys are naked around the female staff, their embarrassment is quite discernible, which is certainly understandable. But underwear seems to be a non -issue. I'm more embarrassed than they are. Are my perceptions correct? Men just care about being in their underwear?

Some men just believe since they are at the doctors that the staff is used to seeing it anyways. They don't see that the doctors and nurses are used to see men naked and in their underwear but other staff many not. When I got to the doctors, if I'm there for a exam and they ask me to remove my pants I will wait until the exam starts with the doctor or ask for something to cover up with. Unlike most men, I don't like everyone seeing me without pants on or naked. I have no problem letting my doctor see my genitals or in my underwear. Being that I wear briefs, mainly white briefs or tighty whitiesand I'm only 22, I don't like all the staff seeing me in them. But older men usually have no problem with showing everything for som reason.
