One of my English dons was a brilliant woman who gained much respect from her students. As a scholar and researcher, plus published expert on the classics, she was quite the revered woman on campus. However,her appearance was a unimportant to her, probably because she was so greatly respected in so many other areas. She didn't require affirmation by being told that she looked good.
I suppose it's about priorities.

I would think most people would want to look their best and that would indicate their desire for approval :)
