In recent days, the president’s health and human services secretary said that despite internal concerns and a failed test run Mr. Obama was not told about serious problems with the new program’s website until it was rolled out this month. Other officials said the president was not aware that the National Security Agency was tapping the phone of Chancellor Angela Merkel of Germany and other friendly leaders until this summer, although intelligence officials said Tuesday that others in the White House had known.

Opposition lawmakers and pundits have seized on the White House explanations to accuse Mr. Obama of being a “bystander president,” as the Republican National Committee put it. Even some Democrats are scratching their heads at the seeming detachment from significant matters. MSNBC’s “Morning Joe” ran a montage of clips showing Mr. Obama or his aides disclaiming presidential knowledge of various issues as well as a graphic titled “Implausible Deniability.”

“It seems to me there’s a pattern here — with any bad news coming out of the administration, the excuse is the president just didn’t know about it,” said Representative Adam Kinzinger, Republican of Illinois.

“There’s a point at which the I-didn’t-know excuse really violates the idea of the buck stops here,” he added.“We want to have a feeling that the president ultimately takes responsibility. The American people want to know they have a president who’s in control and in charge.”

Democrats were less likely to blame the president but suggested that he was ill served if other officials did not keep him fully abreast.“If people really knew there were to be problems, I was a little surprised that people at the highest levels weren’t aware,” Patrick Griffin, who was a top White House official under President Bill Clinton, said of the health care program.

As for the N.S.A. surveillance, Senator Dianne Feinstein of California, the Democrat who leads the Senate Intelligence Committee, put it sharply in a statement she released earlier this week.“It is my understanding that President Obama was not aware Chancellor Merkel’s communications were being collected since 2002,” she said.“That is a big problem.”

Aides dismissed suggestions that Mr. Obama did not pay enough attention in either of these areas. On the spying program, they said the president was deeply immersed in details of the nation’s surveillance practices but was focused on those areas that constituted the major threats to the United States. He had no reason to suspect that Ms. Merkel or other leaders of close allies were being tapped, nor did he think to grill anyone about it because that was not a high priority, they said.

On health care, aides said that Mr. Obama had been fixated on details of the law’s carrying out and that advisers did not withhold information but were likewise surprised by the scope of the problems.

Separating the day's myths and realities on Obamacare

Developments in the rollout of Obamacare are coming with dizzying speed, though not as fast as the pileup of fiction and misunderstanding created by politicians, pundits and the news media. So here's a list of the latest themes you're hearing on America's healthcare reform, and what they mean.

1. Obama "knew" that people would lose their health insurance. This story, chiefly promoted by NBC News, reflects the Washington media's eternal search for scandal, abetted by every politician's instinct to reduce even the most complicated ideas to a sound byte.

It was always clear that many insurance policies serving the individual market wouldn't conform to the coverage requirements set by the Affordable Care Act and would have to be changed. Some were "grandfathered" in, but the rules dictated that any that were changed by the insuring companies -- including changes in premiums or other terms -- would lose that status.

As a result, millions of policyholders are now being informed that their nonconforming policies are being canceled as of Dec. 31. The idea, of course, is for them to get new policies under Obamacare as of Jan. 1. NBC is breathing heavily over its investigative "discovery" that "because of normal turnover in the individual insurance market, 40 to 67% of customers will not be able to keep their policy" mostly because they changed plans.

But is this news? No: The exact same figure was put out by the Obama Adminsitration -- in 2010. Here's a release from the Department of Health and Human Services from June that year, explaining that "40% to two-thirds of people" in the individual market normally change plans in a year, and thus would no longer be in grandfathered plans. Did Obama "know"? Yes, but so did anyone else who was paying attention, including reporters covering healthcare.

U.S. Departments of Health and Human Services, Labor, and Treasury Issue Regulation on “Grandfathered” Health Plans under the Affordable Care Act

Most of the 133 million Americans with employer-sponsored health insurance through large employers will maintain the coverage they have today. Additionally, large employer-based plans already offer most of the comprehensive benefits and consumer protections that the Affordable Care Act will provide to all Americans this year – such as preventing rescission of coverage.

The roughly 42 million people insured through small businesses will likely transition from their current plan to one with the new Affordable Care Act protections over the next few years. Small plans tend to make substantial changes to cost sharing, employer contributions, and health insurance issuers more frequently than large plans. To help small businesses afford employee coverage, the Affordable Care Act includes a tax credit for up to 35% of their premium contributions.

The 17 million people who are covered in the individual health insurance market, where switching of plans and substantial changes in coverage are common, will receive the new protections of the Affordable Care Act sooner rather than later. Roughly 40 percent to two-thirds of people in individual market policies normally change plans within a year. In the short run, individuals whose plan changes and is no longer grandfathered will gain access to free preventive services, protections against restricted annual limits, and patient protections such as improved access to emergency rooms.
