Dear President Obama: I admit I am not up to speed on some of your health care plans. But I am intimately familiar with health care, being a physician and a taxpayer. It is for this reason that I invite you to join me at work for a day at Stroger Hospital of Cook County.

You will immediately notice that as many as 40 percent of the patients are illegal aliens who are getting free care.

Who pays? I frequently see patients who fly in from other countries to get free care.

Recently, we had someone fly in from Africa. I do not believe he was a citizen or legal resident either. It is no wonder Todd Stroger, our County Board president, has had to raise county taxes?

Although I am no fan of his, all the problems here are not his fault. A large percentage of the patients, be they legal or not, have what I call "diseases of self-abuse." People are morbidly obese, doing drugs (especially cocaine), never exercise, smoke, drink and generally are responsible for many of their own health problems.

Again, the question is who pays.

The vast majority of our trauma patients were not participating in a church choir when they got shot.

Who pays for the police and the health care?

Oh, I left out the judiciary system and jails.

Is it no wonder that I was told by a co-worker they had read somewhere that 60 percent of people get more from the government than they contribute? There is a difference between providing a safety net and actually encouraging irresponsibility. Living in Chicago, you and I have both seen the lack of effect all the social programs and affirmative action have had.

Should I mention gun control here, too? We have as much if not more crime.

We are not solving the problems but encouraging poor behavior and the lack of accountability and responsibility. We need health care and public services, but we have a situation where fewer and fewer people are providing more and more for others.

You are right: Health care is a mess. The problem is that to a large respect, the government shares the blame.

Having more government involvement may be like putting another hole in a boat to get rid of water coming in. The more the government puts people in plans that may pay even less of the cost of the care, the more the costs are shifted to the remaining people paying.

Pretty soon, the remaining people can't afford it either.

A co-worker of mine has a sign in his office that is appropriate, especially given that he is from the former Soviet Union.

It reads, "Socialism is great until you run out of other people's money."
