Fecal Digging - Dementia?

Hello - My grandma has started "digging" her at her fecal matter. She has been living on her own just fine. Recently, in the last four weeks, we have noticed a decline in her health. She has now started digging at her fecal matter and insisting on going to the bathroom with certain family members are her caregivers. She only does this with two of my aunts. When the rest of us care for her (we are staying with her, rotating the caregivers) she does not do this. The result is horrendous with fecal matter all over the toilet and walls near the toilet. It has come to our attention that she may have engaged in this behavior for at least a year prior to the recent health issue. Please advise as to if this is common in the elderly or is this dementia. I am having this same problem with my Mother that has Alzheimer's. Please help. This is the worst thing that I can ever imagine. Is there anything we can do to prevent her from doing this????
She is getting fecal matter everywhere.
