VA hospital makes changes after fatal beating
The Truman Memorial Veterans' Hospital failed to provide a safe environment for a 78-year-old man who was beaten to death last year at the Columbia facility, according to a VA report.

Probably not the first time. The VA's leadership believes it is too good to associate with the communities that surround their Medical Centers so no information is exchanged. They also have many regulations in place to prevent open communications. This article also refers to the "VA POLICE OFFICER", just how many Officers were on duty? Makes it sound like just one. Any lawyers out there? This one sounds like a slam dunk for some big bucks.
