You are so very right on with that last comment. I will go as far as to say this. My clinic is in Vonore. I don't know why this forum says I'm from Chattanooga :/ My clinic is legit. I just spoke with the local DEA agent yesterday (as I was also turning someone in.) He is a good man and the DEA has a really hard job to do. But there is no reason legit clinics can't work hand in hand with them to make sure the people that are not legit get caught and prosecuted. I will turn in anyone I even suspect is selling or abusing. But I try to take care of my legit patients...as best Ic an :)

Yeah, it means you can't get through med school. It also means that you make a living selling drugs to users.

There are many good facilitys out there just call ahead and ask questions like .. do you take referals .. do you have a MD on site things like that ... Prefered Pain just opened up off john sevier hwy and I know for a fact they require referals .. a primary doc and office notes from your visits... but yea just look aorund an always ask questions

A great many of these are under review and investigation, from what I have read. Many are run by some low class docs and shady managers. It's understandable why they have a bad reputation.

Well then they are the ones giving most a bad name... sucks that most of you all beleive everything the media puts in the papers.. think about it cigerrette companys have far more people addicted to them than people addicted to pain pills but you dont see gov or anyone cept freedom groups trying to go after them for giving people cancer ... all in all just ask questions if your PCP refers you to pain management

I don't know why certain people keep targeting you & other poster big T with mean & immature remarks. Almost every med practice has CNP or FNP working under the MD. Last year, a NP saved my life because I had an obstructed gallbladder & the ER MDs didn't know & didn't care but she got me to a good surgeon in time.

Yes we all know the shady clinics are giving the decent clinics a bad name. Other than the fact most PMD do not want to write for chronic pain, most do not know all the info that a good pain clinic does. I found out during my fist visit that 2 of non pain meds I was on was not the proper dose and was ineffective (one was Neurontin which is for nerve pain).

And before I get the remarks - you're just a pill head - let me tell you I do not even take 1/2 of the pills that is prescribed to me because I only take the correct doe when I'm in pain. I injured myself saving a patient's life and I've done everything there is to not have to take pain meds but when you cannot barely walk then no one needs to judge me. Not only did I lose a job I loved because of my injury, I lost over 1/2 my income. I could easily sell my extra pills (like 1/2 of my family does) but I will NEVER do that!

A good pain clinic also tried other methods for pain control (in my case epidural) and does monitor their pts and turns in ones they suspect.

Keep up the good work Pain Sufferer - I could not imagine the tough time you had going through NP school with small children. Nursing school was enough for me! Know you're making a difference in the lives of the suffering and getting the bad ones out of the system!

And bigT - I hope you find a decent clinic. No one deserves to chronic pain & be judged as harshly as you have.

i commend you my friend.and i to was hurt on the job,i was a line man for 11 and a half years,a job i loved so very much and made very good money at.and like you say when i got hurt i lost half my income too.and i do understand it is hard to get insurance started in business,i have medical insurance but there are clinics that are not set up for insurance yet,and i do understand that.and if i were a business as such i would only accept certain insurances too,becaus some are very slow paying and some dont pay at all.and seriously business is business,and charity wont pay the bills.and to painsufferer again i commend you my friend.JOB WELL DONE.I HOPE TO SEE YOU SOON.....

I could have gone through medical school had I WANTED to. It was my choice not to go. I found I could make a bigger difference in nursing. I have no regrets and absolutely love what I am doing. It was hard going to school with three babies but I wanted to show my girls that I could and thus they could make a difference in this world.
Bit T - I wish I could give you the clinic information on here but I do not want to abuse the forum. I am not on here to advertise..only to educate :) Hope to see you soon too :)

I do know to which I speak, but all the below is only the opinion of the writer--

The nurse practitioners have to follow the guidelines that direct the use of scheduled medications such as opioids, benzos, and other dangerous or addictive medications as listed in the Tennessee Board of Medical Examiners policies and rules. They only have to be supervised remotely with the physician visiting the clinic once a month and signing the charts every ten days. This can be performed electronically.

In training, nurse practitioners receive 2 hours of pain management training and 2 hours of migraine treatment training. That is all. None of them have ever had formal training in spinal procedures. They did no, notta, zilch procedures in their training, and no formal, cadaver based courses are available for post graduate work.

Any education in chronic pain management comes from their on the job training after nurse practitioner school. The quality of training depends on the physician who trained them. If this training came from primary care physicians with no specialty training (addictionology doesn't count) or from physicians who have lost their license or have been arrested for fraud (McNeil and Foster for example,) the levels of training may not be at the best . Some doctors are simply too sympathetic to their patients to say "no," even if it is in the patient's best interest.

I do not blame the nurse practitioners who have been either inadequately or improperly trained. Most are doing their best to help people who claim to have pain. However, one of the easiest ways to fill a waiting room in East Tennessee is to hang a pain clinic shingle and let the word get out on the street that you are an easy mark. The folks out there who want the medicines for the wrong reasons will always be better at fooling you than you are at detecting them, especially if you do not know what you are doing or the true dangers of the medications or procedures. I am afraid that many of the pain practitioners out there do not know that what they don't know is the key to proper treatment.

The 6-keto opioids (oxys, roxys, hydros, and diaudid) are highly addictive medications that stimulate the pleasure centers in the brain when there is a peak in blood levels. Roxys and hydros along with crushed OxyContin are immediate release preparations that will deliver that peak in blood levels. The high, anti-anxiety, sense of well being that accompanies these peaks is why many people will say that only these medications work - they can feel them work in ways other than pain control.

Therefore,(you knew this was leading somewhere) best current practice for chronic intractable pain management requires a formal psychological screen (the ORT is worthless) for likely addictive behaviors or misuse of the drugs, a diagnosis backed up by tests (over 60% of people without back pain have at least one abnormal disc, everyone over 40 has degenerative changes in the spine and do not qualify,) non-addictive means must have failed or not be indicated, a written treatment plan be developed, and proper written monitoring be performed.

All of these can be in place, a medication agreement signed, urine drug screens performed and the prescription monitoring checked and the practice still be a pill mill. These items are mere window dressing on the building props of a movie set. It is the knowledge behind the scene that makes for proper care.

I do not know how to describe a pill mill, but some aspects are: 1) cash only practice, 2) high percentage of patients on OxyContin, 3) the use of Roxicodone 30, more than 120 short acting pills a month, the frequent use of Soma, frequent use of Xanax with narcotics, or walk-in patients receiving medications on first visit.

Chronic pain is not simply acute pain that has lasted a long time. The psychological impact is similar to the death of a loved one (the old self.) The spinal cord and brain change. True expert care is needed.
