I would be interested to read this book as I am a long time TMJ sufferer. I go to a chiropractor and the assistants use the ultrasound machine on my jaw area but it doesn't seem to help. I have tried some exercises. I'm curious to know why food would be a factor. I currently wear a hard acrylic night guard that I had made for me through a company online. At least I have that. It definitely helped with some of the jaw pain and the migraines, and it prevents me from cracking my teeth. is the link to the night guard company (if anyone is in need of a sleep guard). They were really great and way less expensive than the dentist!
