"The insurance industry is fighting the effort, saying such mandates drive up costs and reduce the flexibility customers want."
What a pile of BS. I needed a simple shoe insert with a price tag of $400 that my insurance company would not pay. I paid for it out of pocket and the prosthetic has worked miracles in curing my back pain. However the insurance company was willing to dish out thousands if not tens of thousands for the alternatives offered to me.... steroid injections, pain meds, and disc surgery.
