Wark is right. It was more a failure of human systems and the only way to avoid that going forward is to close this old, delapidated plant and move on. Finding the leak(s) was a good thing. Firing a few liars (don't worry there are plenty more in this company to go around) did little.
Enough of this less than honest company. Time to move on. Before all you pro-VY folks go crazy you should go on-line and do a search on Entergy. Then get back to us

Entergy did what they said they would in a timely fashion. The leak was from piping installed prior to Entergy owning the plant. The leak would have occurred regardless of any statements made about piping. Bottom Line... Entergy has always maintained a safe , reliable plant ...... There are alot of hard working people at Entergy Nationwide who contribute to this Country..What are you trying to contribute?? Ask yourself??
