This is tragic and extremely sad being it's a child. I

For the entire gate to come off at once tells me it wasn't mounted correctly. You would figure it would start as slow rip away from the wall. My guess it was a combination of improper mounting and a deteriorating wall or frame. Bolts and anchors should've held part of the gate. It almost sounds like they didn't secure it enough perhaps chincing on the number of bolts.

Saw a picture of a nail on the news last night.

Nails are used to attach/connect but usually NOT to hold a load. No matter how long a nail it won't hold a heavy load like a bolt, nut and washer. And the only reason many codes want or allow nails is that so something can rip or fall away from a structure without bringing down structure with it. Even then there are usually enough nails so it's a slow rip a way or partial detachment. An example might be a porch roof but porches also have pillars and/or support in addition to nails. Total collapse means something was too heavy for what ever was being used for support.

A moving gate like that has attachment and support issues. When someone closes the gate or pulls it down there are probably outward forces pulling on the frame in addition to downward gravity.

I hope these installers didn't t use a nail gun. Again no matter how long the nail or deep you get it nails simply don't hold weight like a bolts and/or screws with anchors. I wonder if some thought or assumed that if a nail gun was being used it would make it "ok" or stronger. It's the type of fastener and not the tool being used.

They say now they were using lag bolts.

If it was nothing but lag bolts either they didn't anchor them or correctly, went into to shoddy materiel or missed structure underneath. No matter how you look at for the gate to fall all at once with no partial detachment or slowly rip away means every bolt failed at once?-something is seriously wrong,

I hope they release their findings asap because there are a lot of do it your selfers out there.
