Having worked for IP in Pulp, Power, and Woodyard Area's, this is the workers worst fear come true. A Recovery Boiler is like a controled Volcano, at its best. Our prayers are with the survivors and families.

What was the name of the contractor that employed the employee's that were injured and killed in the IP mill explosion?

Recovery boilers can be very dangerous beasts if water tubing in the walls or in the generating section (tubing that connects the mud and steam drums)ruptures, dumping water onto the smelt bed, which consists mostly of sodium salts. If enough water hits the bed, the resulting explosions can be devastating.

I worked as a helper on one of those dudes for about 2 years, keeping primary and secondary ports for combustion air open, and keeping the smelt flowing out the spouts from the hearth (bed).

I was one happy person when I walked away from that job for the last time.(It wasn't an IP mill.)

I should have pointed out that the smelt bed consists mostly of MOLTEN sodium salts (under normal operation).

the explosion was due to operator error. they had treid to light it several times and the fire went out. the boiler then filled with gas and ignited. there were no ventilation fans used.

OK....So it wasn't a smelt/water explosion.

If the gas burners on the boiler were being lit w/o the ID fan running for ventilation, very serious safety interlocks were bypassed. I have worked around recovery boilers for 30 years. Boiler purge interlocks require the ID fan being run and that at least 5 boiler gas volumes purge the boiler of combustibles so something like this can't happen. Bulldog, if this is indeed what happened, it is not "operator error". A decision to block boiler interlocks of this nature would require supervision.
