From exalgo pamphlet
5.2 Life-threatening Respiratory Depression
Respiratory depression is the primary risk of EXALGO. Respiratory depression, if not immediately recognized and treated, may lead to respiratory arrest and death. Respiratory depression from opioids is manifested by a reduced urge to breathe and a decreased rate of respiration, often associated with a “sighing” pattern of breathing (deep breaths separated by abnormally long pauses). Carbon dioxide (CO2) retention from opioid-induced respiratory depression can exacerbate the sedating effects of opioids. Management of respiratory depression may include close observation, supportive measures, and use of opioid antagonists, depending on the patient’s clinical status [see Overdosage (10)].

To all others.

We all react differently to everything. I've been on pain meds for 5 yrs now, I can't even remember what I started out on. What I can tell you is, that for me Nucyenta, Opana, and Exalgo are the 3 that helped the most- until they didn't.
I never crushed any of mine, the thought never crossed my mind. I don't believe it would do me any good anyway. My body goes through medicine far too quickly for anything but "er" labeled meds to work effectively on me, for any amount of time at least.

It IS NOT judging. There are those of us that NEED this medication for severe long term pain. There are many more that need it that CAN NOT get it because of idiots that abuse medications like this just to get high!!!!! It is because of these a$$holes that many people must live with pain that much of the time becomes unbearable because their doctors are afraid to prescribe it because they are afraid it will be sold instead of used properly. It is usually people with mild to moderate pain that learn how to fake severe pain just to get a medication that they can sell. It took me two years to find a doctor that would prescribe a medication that was strong enough to deal with my pain. My pain became so bad that one day, since my doctor would not prescribe pain medication and tried for two years to treat my pain with antidepressants which did nothing to help me, that I chose to end my life because I could no longer live with the pain. Then I found the doctor I am seeing now who understands pain. She is very cautious and has very strict rules that her patients must follow, but for those like myself, those rules are welcomed because she listens to us and cares. Personally, I feel that the pharmaceutical companies should take all medications like Oxycodone and Kadian back to the drawing board and re-engineer them to make them much harder to abuse, and maybe one day impossible to abuse.
I wish that the people that abuse medications like this would for just six months become afflicted with the level of pain that I suffer from 24 hours a day, every day so they could see what some people must live with. Some would no doubt understand, but I am sure that many would still believe that it is their right to abuse these medications that many must depend on just to make daily life tolerable.
I am sorry that this is so harsh and to the point, but if you ever suffered with this level of pain on a long term basis most of you would understand and support my position. I hope that you are never in a position where you suffer from this level of pain, but it is sometimes the only way for some people to understand what it is like for many of us every day of our lives.

Hi all. Im new here.... Long story short, i have been living with a lot of back pain. 15 years worth. This last October i had surgery on L4-L5-S1. they put a concrete like substance between my discs, added 4 rods and 8 screws and also dug a nerve root out of L5. I am in more pain now then before my surgery.

My pain management doctor has me on Nucynta twice a day and hydro's for breakthrough pain. the nucynta doesnt help much for the pain. she has prescribed me opana twice a day, which i havent tried yet. i still have the new bottle. and i also have a new bottle of exalgo, which i havent tried. i havent tried either cause im just scared they wont work. i have a high tolerance to pain meds and any other meds. ive been this way for over a decade.

i do like my hydros though. i tend to take more than i should. sometimes 8-10 a day. im beyond tired of being in pain. i just want normalcy. i would be lying if i said i didnt like the high i get from them.

now i need advice..... any and all advice would help.

Hi, To all those out there accusing anyone else of abusing and misusing their medication, WHY DON'T YOU TRY LIVING WITH SEVERE CHRONIC PAIN, BROUGHT ON BY SEVERAL "DISEASES" AND "DIAGNOSES" AND TRYING TO LIVE A "NORMAL" LIFE WHEN IT'S ABSOLUTELY IMPOSSIBLE TO WORK, MUCH LESS ENGAGE IN THE ACTIVITIES YOU LOVE; GET PUT ON EVERY MEDICATION OUT THERE FOR PAIN RELIEF, DEVELOP A TOLERANCE OVER MANY YEARS AND FIND THAT NO LONGER DOES ANYTHING REALLY WORK FOR YOU (INCLUDING ACUPUNTURE, PSHYCHIATRY, MASSAGE, HOMEOPATHIC TREATMENT, ETC, ETC, ETC,) UNTIL YOU ARE TO THE POINT THAT...YOU'RE NOT RESORTING TO STREEET DRUGS, YET...BUT YOU GET NO RELIEF FROM ANYTHING ANYMORE, "UNLESS YOU FIND ANOTHER WAY TO GET IT INTO YOUR SYSTEM AFFECTIVELY:!...AND...CONTINUE TO STRUGGLE WITH ALL FINANCES ON MEDICAID THEN MEDICARE BECAUSE MEDICAID DROPS YOU AND YOU'RE PAYING $479 PER SCRIPT FOR ONE MONTH AND CAN'T AFFORD GROCERIES.....THEN, AND ONLY THEN, AND HOW DARE YOU ANYWAY, JUDGE ANY OF US THAT REACH OUT TO HELP EACH OTHER FIND ANOTHER WAY TO GET SOME KIND OF RELIEF SO THAT WE CAN ENGAGE WITH OUR FAMILIES, FEEL LIKE WE'D ACTUALLY LIKE TO GET UP AND LIVE FOR THE DAY, FIGHT THE CONTSTANT THOUGHTS OF HOW DEATH WOULD HURT SO MUCH LESS, ETC ,ETC,...Anyway, how dare you judge us until you've been through a bit of that?! Don't judge those who take pain med off black market. I don't believe in snorting/shooting up for many reasons. Those people arnt in it captain relief, but that's only my opinion. The others that do black market, such as myself, have to. I have no insurance and am in pain most of my days. I took so much Motrin, it ate up my stomach. I have tried exalgo cut off the grey part and swallowed for instant relief after a fall or something and have taken 1 a day for 5 days once with hydro or Tylenol for breakthrough pain and it made me be able I walk up my stairs without almost crying and I was able to go on walks and play with my kids with I side effects of being impaired. Hydrocodone, even when rx make me feel good but i still hurt. I have a severe knee issue and a bulging disc in my back, but when I started making 8.50 an hour w no insurance, my medicaid was cut. I not even judging the "junkies" as I called them. That's an opinion. As a Christian, I know that it is Gods job to judge us all in the end. I just hope he helps me be able to ,not be pain free, but live w a bearable pain legally soon.

PS

Please pardon my typos in the post preceding this but my Lhasa Apso is up on the bed with me and my keyboard tonight, and making her presence known.:) I think you get the gist of what I had to say about methadone and Exalgo, and that's the important thing. Please let me add just a couple of addenda to my remarks, if I may: the first is Aleve. This NSAID, known generically as naproxen sodium, kicks ass as a neuropathic pain reliever. For effective analgesia it is tops among all the other agents in it's class and I cannot praise it highly enough. Secondly, I have also found marijuana to be an invaluable adjunct to my opiate-based weapons against the myriad effects of chronic pain, especially nerve pain. I live in Illinois, and our state Senate just approved it on Friday after the Illinois House finally gave it the go-ahead a couple of months ago. If you're really hurting and haven't tried it, I would strongly suggest that you do! Thank God (and some kind grower somewhere) for my cannabis sativa!

I got prescribed oxycontin 20 and my insurance wouldn't cover them and the cash pay was 160 for 20 of them. I called my insurance and they said they would cover Exalgo but I was afraid they would be like Morphine pills I got once that didn't work. I came here to see what people thought. I have Wegeners disease and bad inflammation.all I can do is give them a try

Wow, how do you function with your typing skills and what seems to be a serious addiction to pills. He's right, it's jackholes like you that make it more and more tough for us that have serious pain issues to get proper meds that help us get through the day. I don't take them to get high by any means. In fact I very rarely get even a hint of it which is nice because I hate that feeling. But I, and many others, do need these medications to make life sufferable. Idiots that abuse these I think should be jailed..and im not opposed to being shot so they feel what true pain is like. Maybe then you will understand.

And seriously, take some night school in reading and writing.

Hi,
I was just prescribed Exalgo, a once-a-day, time released pain killer that uses a push-pull sysytem to deliver a constant dose of hydromorphone.
Out of curiosity I tried to cruch one without very much luck. I then proceeded to cut the pill in half & found (inside) one half was a VERY TIGHTLY COMPACTED white matter & the other half was a very tightly compacted gray matter. The whole thing is surrounded by what seems to be a pretty heavy-duty plastic-type shell with a pin-point hole in the center of the one side. My understanding is that the "hard candy shell" is passed out through the stool after the drug does its job in your digestive system.
So....doe anyone have any experience with these new Hydromorphone ER pills yet? They seem to be pretty abuse resistant. Just curious what others have experienced.
I've had EXCELLENT results with Exalgo myself. I've tried the patch and Opana. I've taken scores of "4's" and in my particular case the Exalgo and a few Dilaudid work very well!

I was shot in the back of the head with a .45 18 years ago and I take meds for the neuro damage. Specifically Headaches. NOT being on the med "roller coaster" is terrific and Exalgo in my particular case does that. Wake up...feed my service dog...swallow a Exalgo and a 4 and I'm good! Love it!

I have my first RX of Exgalco, and although I am
Opiod tolerant, I am fearful of getting off of Fentyal
patches and starting this. I have had severe back
and neck pain due to spinal degenerative disease
and have been nearly invalid for the past three months since patches started not working. I tried
Opana, and it made me feel good mentally and as
long as I didn't do anything but could not work because of the pain. I have been somewhat prone to depression and wonder if Excalgo will make it worse. I don't have anything but Prrcoset
for break through. Please, somebody reassure me about this.

Hey there - just got prescribed Exalgo (doing mail order) so I'm reading up on it. I'd be afraid too - 44 years of pain, but I've learned that some things work, some don't. Meds will change for you over time-often I felt like a guinea pig since the'70's - so many different meds, surgeries, bedrest. Depression comes with the territory when our lives are just torn from us and we suffer. Can't be productive or enjoy life like everyone else; we are dependent on others, etc. I've done some crazy things, still do actually but if your hands are okay, try anything...art (I started with pencils), I crocheted, I wrote stories - this was all before computers were everywhere. Don't know if you're male/female - it doesn't matter but find something that makes you feel good about yourself. Many times I tried things but couldn't finish it; motivation is often an issue for me. Keep challenging your brain while getting yourself distracted from the pain & boredom. If you can, move in any way you can. I always feel worse when I'm stuck in bed a long time. Whatever you do, get creative.
Be honest & open with doctors (lose the weird ones!). Sounds like you're in this for the long haul like me but I've had 40+ yrs of dealing with this. Fentenyl is strong-primarily used in hospitals until recent years. Changing meds is not a bad thing - some work better, some differently or not at all. Stay with it long enough though - hopefully you get a good break from the suffering and that's what we all look for. Good luck!
