Sad news indeed, the center should be investigated and those responsible should, of course, be terminated.

As to the others, so you think we should just leave them all suffering and killing themselves, VA hospitals are for veterans, the military folks, who earned their right to such benefits by risking their lives for your freedom.

Young people thinking about serving their country should know that if they're injured and become disabled their S--T out of luck .
As soon as you're separated from active service the government will wash their hands of you.
You'll be at the mercy of those who feel sorry for you .
V A employees are like the rest of the federal government just pulling down a huge pay check to set on their ass and complain about how hard they have it. Don,t trust this lying government to help you if your disabled while serving your country . You'll be on your own.

Sad, but probably true as well. If you don't have a large inheritance I guess you shouldn't sign up. Oh well, there goes our military completely.

I have a hard time getting my 'give a hoot' meter off zero for junkies that exist to stay high, but are considered 'suicidal' or 'depressed' if they can't get enough drugs to stay as high as they want. Maybe some emphasis could be expended to jail the drug dealer that provided a high enough dose of chemical high to kill someone.

An investigate needs to be done to find out if the equipment was not held to sanitary conditions BY THE STAFF, including the doctor or nurse. THEN they should be held responsible and this story should be put nationwide to EXPLAIN why this is a constant problems in a VA hospital.
Most of the time, it is due to the laziness of the staff...(I can't think of any other reasons).
The Hospital Director should be investigated and Joint Commission involved in accreditation of this hospital. Of course the possibility of cover up is involved but there are patient there that can speak out and that is what needs to be done here. Maybe some of the employees know something.
I know when my staff balked at things that were going on and no one would listen, we got the newspapers involved reporting the corruption that was going on. That caught the attention of the hospital director and an FBI investigation was done (3 times, due to corruption in the process) but eventually this exposure found three employees responsible and they were sentenced.
It takes a lot to break the govt cover ups with federal govt but speak up load and clear without fear, be prepared to move on to another job and speak up for the patients that can't!
