City of LA Mayor has been encouraging people to use public transportation. It appears periodic safety reviews were not done.

Qualified planners could eliminate the factor of human dependency and implement other measures. What were they waiting for?

SO.... WHEN, are they going to release the name of the driver that caused this horrific crash? Are they holding back because there's something about the deceased driver that will be an added outrage?
