I heard that a person was reported missing and police went to Intat and they found the person dead from some kind of accident. Wish Tv has some info on their website, but no names or anything.

This is too sad. I know most folks think that on the job safety measures are a pain in the ass but the alternative is worse. Being maimed or losing your life is not worth ANY salary. Wear your P.P.E. and be constantly aware of the environments you are exposed to. Always follow lock out tag out and confined space procedures to the letter Never do anything that puts you or others at risk of injury. If your job is unsafe let your supervisor know and ESCALATE the issue if your supervisor refuses to listen or fails to resolve the issue.
In 2011 4,609 workers were killed on the job. Over 4,000,000 were injured. Do your best to work safely, you owe it to yourself and your loved ones.

This is Rushville. The rumor mill is running rampant with the name of the victim.
This news even made the Indianapolis TV newscasts:

But if you people really want to know the name of the victim, get in contact with the current Rush County coroner, Ann Moster. And probably coroner-elect, Christy Hall, can provide you with the name.

The person went into a machine 2 - 3 am (Perhaps to clean it).

The control room turned the "muller machine on" they didn't know that he was in a piece of equipment.

Unconfirmed and Rumor-ish ... so don't take it for fact until an official report comes out.

I understand the victim was a "green striper", meaning he had been working there less than ninety days. If this is the case I certainly hope the company has documentation of lock out tag out training for him. If the Muller was properly locked out the control room personnel should not have been able to start the machine. Lack of training? Safety procedures not followed? An investigation will ensue an the cause will be revealed. The bottom line is a young man clocked in and did not make it home to his family that loved and depended on him. It could happen again on any day at any time. Do not depend on others to do the right thing. Working safely is up to you so please, do not take anything for granted.

After working in a factory for several years it is very apparent that someone did not follow safety precedures and IOSHA Laws. How could somone not know this man was missing? Why was he working alone inside of a machine? Was this a 'Confined Space' violation? Why was it not 'Locked Out'? How many people have been seriously hurt at this plant? I believe this is the 2nd person to die at INTAT. WHY?
