Halliburton pumps whatever cement BP plans. Halliburton doesn't say how much or what weight or what type. Drilling consultants are (should be) on the cement unit while the job is being performed to verify proper weights and volumes, plugs bump etc. As for the centralizers, we will never know if more would have helped or not, but I believe Halliburton did recommend them.
BOP's should have been closed long before explosion. The signs were there as you state with the negative test. As I stated, they failed to recognize and respond to what the well was telling them.
The OIM has the final say on the rig, including stopping any unsafe BP practices. The OIM is responsible for the safety of everyone on that rig.

Great write up. However, I believe the BOP must now be modified to self actating in the event of safe parameters are exceeded. The risks and proven devistation requires a mechanical overide for human failure/oversight. I believe that will be one of the fial recommendations by the Coast Guard and Congress.

It would be nice to have something like that, but I believe there are too many variables that go on while drilling a well. If the system you are talking about could be manipulated for different practices, it could then probably be set to ignore safe parameters, therefore we would be back to human failure/oversight.
