If an adult had a urinary infection and refuse to take antibiotics to treat it in hopes that they would die is there anything a spouse or therapist could do to intervene? And how long would it take?

You don't provide enough information to receive a legitimate answer. What is the patient's normal cognitive status? How old are they? Is it possible the UTI itself is hindering their ability to make a rational decision (number 1 cause of confusion in the elderly is a UTI)? Does the patient have a symptomatic infection or just a chronic infective urine specimen from having an indwelling catheter? Is the patient a hospice, palliative, or end-of-life patient that does not want aggressive treatment of illness unless they have pain/symptoms?

Every ethical dilemma is a distinct case with distinct circumstances.

I am a nurse. Just like I said, "in an otherwise healthy" adult, meaning an adult with a normally functioning immune system, a urinary tract infection is not deadly. In fact, the only reason to treat it at all is because patients don't tolerate the symptoms. In an otherwise healthy person, the vast majority of isolated illnesses can be fought by the immune system. If we would stop running to the doctor and ER for every sniffle, we could probably still afford healthcare.

A UTI can progress to sepsis which can progress to septic shock which is life threatening
