I'd be willing to bet that if people crossed where they are supposed to (i.e. crosswalks and overpasses)instead of wherever they wished, that there would be a lot less fatalities.

I blame the idiots that set the timing of both the cross walk signs and the stop lights. Ever notice that the cross walk signs should turn green first at least 30 seconds. Then the red lights for the cars turn green.
I also blame drivers who feel the need to drive as if they paid for the road them selves. I drive and also walk to and from places and I've seen plenty of idiot and courteous drivers. But then it just might be fun for the idiot drivers to get to the night red light first.
