Medical Insurance and Emergency Room

I have been very sick for nearly two weeks now. I have medical insurance, but I can't afford the deductible the Doctor's office and the aftercare places want to collect up front. If I go to the emergency room, will I have to pay up front or will they bill me so I can pay what the insurance doesn't cover later? Not trying to get out of paying, just hard times. My husband and I pay 850 a month for us and our daughter.
