After being involved in a near-fatality, this engineer and crew should have been pulled from service. The second incident could have been prevented if the company had pulled him.

But how can it be the engineers fault if a car pulls out in front of the train?

Yeah, it's not like a train can VEER off course and run into a car. The tracks are pretty much set. Seems like too many people aren't obeying the signs when a train is coming. Sad that 2 had to die for it, though. :(
