Why is it so hard for someone to keep a six foot wide vehicle inside of their own 11 (or even 12) foot driving lane?

I suppose those will always be factors, but I see this on a daily basis to and from work. People either constantly drifting to one edge of the lane or another, or simply taking corners too short (i.e. crossing the yellow on a left curve, or the shoulder line on a right).
After watching that, and how close some of these bikers hang on the yellow, I can see it being a factor in a lot of motorcycle vs. car accidents too.
