A Rural Hospital Is Closing In Tennessee Because The State Refuses To Expand Medicaid
A rural hospital located in Brownsville, Tennessee is ending its inpatient and emergency services this summer because it can’t afford to keep operating them. Instead, the facility will become an urgent care clinic dealing with minor illnesses. W. Larry Cash, the chief financial officer for the community health group that operates the hospital, told the Tennessean that his state’s refusal to expand Medicaid was a “contributing factor” in the move.
Providers that serve a high number of poor and uninsured Americans, technically called “Disproportionate Share Hospitals,” often operate on a loss because their patients can’t always pay for their care. To compensate, the federal government offers reimbursements for those hospitals — but the Affordable Care Act changes the way the payments are structured. Because the health law intended every state to expand Medicaid, and therefore reduce the number of uninsured people who can’t pay their bills, the reimbursements for DSH hospitals have been reduced.
But if hospitals are located in states that continue to refuse Obamacare’s Medicaid expansion, that puts them in a difficult spot. They’re losing out on some of the federal government’s funding without making up the difference with an influx of insured patients.
This is by no means an unexpected consequence. For years, rural providers have been warning that failing to expand the public health program will put them in jeopardy. Last December, the Tennessee Hospital Association predicted that rural hospitals would be forced to start closing unless the state reversed its decision on Medicaid. The Rural Policy Research Institute sees the fight over this particular Obamacare provision as one of the most important issues facing its constituency.
Nonetheless, GOP-led states have remained steadfast in their opposition to Medicaid expansion. Rural hospitals are beginning to close in states like Georgia, North Carolina, and Virginia. These facilities are often located in areas where poor residents desperately need access to health services.
In the states that have agreed to expand Medicaid, some rural health clinics are closing for an entirely different reason. In Arkansas, where GOP lawmakers struck a deal with the federal government to implement a state-tailored version of the expansion, a free health clinic just closed because so many people now have insurance coverage under Obamacare.“Our services won’t be needed anymore, and this will conclude our mission,” the clinic director explained.

A new Census Bureau study shows that the Republican-voting south is feeling the effects of the recession more than the rest of the country.
The number of people living in poverty areas, or areas where at least 20 percent of the population is living below the federal poverty line, in the south increased by nine percent between 2000 and 2010, going from roughly 21 million people to over 34 million. This number is alarming, given that the northeast only saw an increase of 3.3 percent, going from around 2.7 million people to just over 3.4 million.
Out of the six states with the highest increase in poverty, five of them were in the south: North Carolina (+17.9%), Tennessee (+16%), Arkansas (+15.7%), Georgia (+14.6%), and South Carolina (+14.4%). What this means is that nearly a third of all Southerners are now living in poverty areas.
While these numbers are depressing, they should not come as a shock. The south currently leads the nation in not just poverty, but in teen birth rates, cigarette smokers, and obesity. It falls behind in wages, access to healthcare, and economic mobility, all of which cost the states, and the federal government, millions of dollars.
Out of the 10 states most dependent upon the federal government, half are in the south. Mississippi ranks number one, with over 45 percent of its yearly revenue coming from federal funds.
These numbers make it obvious that the Republican party is hurting the south. With their tax cuts for the wealthy, destroying safety nets for the poor, and reducing funds to educational programs, the GOP has managed to make life much, much worse for those who consistently vote for them.
Conservatives have hurt the south for years by convincing its residents to vote against their own interests. If they are able to expand their faulty policies into the rest of the country, the damage done will most likely be irreparable.

In fact, desperate Central American parents are exploiting separate legal loopholes in American border security passed before Obama took office.
Unaccompanied minors fall under the bipartisan law, William Wilberforce Trafficking Victims Protection Reauthorization Act of 2008, which passed the House and Senate unanimously and was signed into law by President George W. Bush.
That law says the children cannot be sent back. They must instead be held humanely by the Department of Health and Human Services until the courts release them to a “suitable family member” in this country.
The child “shall be promptly placed in the least restrictive setting that is in the best interest of the child,” the law stipulates.“Placement of child trafficking victims may include placement in an Unaccompanied Refugee Minor program … if a suitable family member is not available to provide care.”
Department of Homeland Security (DHS) sources say more than 80 percent of these children will find permanent homes in the U.S., with either family or foster homes and not be sent back to Central America.
Meanwhile, there is no place to house those mothers flooding the border with their children, as Thomas Homan, executive associate director of ICE for Enforcement and Removal Operations, told a House committee last week.
