You obviously have no idea how the Oil and Petrochemical works.
IF this is what happened, sorry it is still Dow's responsibility and "fault" for not insuring it's employees follow procedures.

I can't stand chemical plants but it's quite a stretch to accuse them of killing an employee. Many employees disobey procedures and hurt themselves. If you can't prove your allegations then shut the hell up nimrod.

Third party (OSHA) cited DOW in this case at least that's what the article said. If the employee violated procedure it didn't indicate that. Can you prove he did? Yeah shortcuts and avoiding procedures can and often does lead to injury or death but theres nothing here to indicate thats the case here. Either way, tragic to die at work

Even though an employee may or may not have followed a procedure the employer is still at fault for not providing a safe workplace hence the OSHA fine (if indeed they were fined" Had the employer installed an engineered device that ultimately eliminated human error then this might have not occurred..might..Not knowing the specifics of the incident, but understanding OSHA general and Construction industry and PSM.
