A non-surgical approach to the management of lumbar spinal stenosis: A prospective observational cohort study

Background
While it is widely held that non-surgical management should be the first line of approach in patients with lumbar spinal stenosis (LSS), little is known about the efficacy of non-surgical treatments for this condition. Data are needed to determine the most efficacious and safe non-surgical treatment options for patients with LSS. The purpose of this paper is to describe the clinical outcomes of a novel approach to patients with LSS that focuses on distraction manipulation (DM) and neural mobilization (NM).

Methods
This is a prospective consecutive case series with long term follow up (FU) of fifty-seven consecutive patients who were diagnosed with LSS. Two were excluded because of absence of baseline data or failure to remain in treatment to FU. Disability was measured using the Roland Morris Disability Questionnaire (RM) and pain intensity was measured using the Three Level Numerical Rating Scale (NRS). Patients were also asked to rate their perceived percentage improvement.

Results
The mean patient-rated percentage improvement from baseline to the end to treatment was 65.1%. The mean improvement in disability from baseline to the end of treatment was 5.1 points. This was considered to be clinically meaningful. Clinically meaningful improvement in disability from baseline to the end of treatment was seen in 66.7% of patients. The mean improvement in "on average" pain intensity was 1.6 points. This did not reach the threshold for clinical meaningfulness. The mean improvement in "at worst" pain was 3.1 points. This was considered to be clinically meaningful.

The mean duration of FU was 16.5 months. The mean patient-rated percentage improvement from baseline to long term FU was 75.6%. The mean improvement in disability was 5.2 points. This was considered to be clinically meaningful. Clinically meaningful improvement in disability was seen in 73.2% of patients. The mean improvement in "on average" pain intensity from baseline to long term FU was 3.0 points. This was considered to be clinically meaningful. The mean improvement in "at worst" pain was 4.2 points. This was considered to be clinically meaningful. Only two patients went on to require surgery.

No major complications to treatment were noted.

Conclusion
A treatment approach focusing on DM and NM may be useful in bringing about clinically meaningful improvement in disability in patients with LSS.

We'll get to the chiropractic claim that nucca and activator techniques can change spinal canal narrowing a little later.

Try to focus mr. chiropractor. Why is it that while thousands of chiropractors claim that they can cure/treat bed wetting, you are unable to?

Then we'll go onto which chiropractic technique is best for treating "spinal stenosis". I'll allow you to post the chiropractic studies which show which technique is most effective. Indeed, I'll allow you to post studies which show chiropractic is better than placebo.

Oh wait....there those studies don't exist. Sorry, back to your inability to cure bed wetting. Why is that?

Let's see if you can answer simple questions: What qualifies you as an expert on chiropractic? Do you have a DC degree? If so, are you licensed? If you are a licensed chiropractor, or ever have been (I'm giving you an out), then have you ever practiced?
These are basic questions that anyone with any integrity and nothing to hide can answer with a simple response of 'yes' or 'no'. Let's see if you can answer these questions. This ought to be good. No reponse means you're a fraud. Anything but a yes or no answer also means you're a fraud.

I love being a Chiropractor and my chosen field. Logan Chiropractic College was founded on Light Force Techniques and is the premier college for Light Force Chiropractic. I based these findings on my Patient results. Yes, some techniques work better than others however, I only incorporated the therapies that are the most effective and that can be achieved with as little stress as possible. I have nothing against standard adjustments and in some cases, it can address issues that I can not through light force, which is rare. If that is the case, I do whatever is necessary to get my patients the best results. My loyalty is with my patients, not my technique.

Ditto. I also enjoy being a chiropractor. What a fantastic field! I incorporate some low force techniques, but I also use spinal decompression to treat disc cases. My patients get excellent results and are grateful they don't have to go through surgery. Of course there are some I do refer to an orthopedic surgeon, but very few need it. And the pay isn't bad, either!

First, let me say that you obviously are looking to argue a point that makes no sense and your attempt to label Chiropractic as a fraud is weak and without merit. Yes, all treatments work, just not on everyone because, like in medicine, people's spine respond differently. We are not made of medal, springs, nuts and bolts. Our bodies are unique with a wide variety of issues which effect our spines and overall health. Calling me a "quack" without taking the time to read this information with enough comprehension to even spell my name correctly shows me that you are not attempting to understand, just trying to be snide and disrespectful to pass the time. The much larger point you miss Mr. Wisdom is that chiropractic helps millions of people everyday. I love my field, my patients and I sleep very well at night knowing what I do makes a difference. I am not sure helping people is a priority to you and your life but it is for me and mine. So go ahead, your turn to blast some cute remark or insults but it won't change the truth or facts you chose to ignore for petty one ups man ship. I sincerely hope that one day you will ignore your own misconceptions, make the leap of faith and see a chiropractor. Who knows? The problem is that your brand of chiropractic is based on nothing. Placebo maybe. Nothing more.

You are clearly unable to address all the fraud within your field. All the bizarre techniques that chiropractors use such as your idiotic "logan basic". As you know, Logan quackery school teaches that subluxations cause disease. That is outright fraud and without merit.

You now that thousands of chiropractors believe that by ONLY tweaking alas, they can cure disease and illnesses.

I do love your use of a talking point taught early in chiropractic education. "Chiropractic works, it just might not work for you".

Mr. hendon, you need to clean out your chiropractic diaper. And help weed out the crap in your field. But you won't. You are a chiropractor after all. Unable to criticize any chiropractor or technique. You are intellectually incapable of being honest.
