I can completely agree that there should have been better safety precautions in place. Their "all clear" signals are simply inadequite.
But the part that sickens me the most is that authority that is supposed to "certify" amusement rides and attractions did not deem it necessary to have more than a safety harness and hand signals to prevent this kind of an accident.
People get complacent for whatever reason and it is the responsibility of regulators to ensure that there is little, if any, chance of human error causing such a mishap.
The operator made a horrible error, but the local regulators did too when they allowed such an attraction to be used without more elaborate failsafes.

The insurance company who covered this ride should of been all over the method they had for giving the signal to drop the rider on this ride. I can not beleive this got by them and was allowed to operate like it did. I can understand the operator saying he blanked out. If you have to do something like this time after time as many tims as he has for a whole summer. It would be hard to stay tuned in 100% every time and not have yourself wander off a little bit once in a while. This does not excuse what he did though. He should of been able to see these times and took a break or shut down the ride till he got his sense back about him.
