The plants at Fukushima exceeded design capability, the problems were caused by operator error, something Tepco has admitted to.

Next generation designs rely even less on operator interference.

In the end, one person was killed by the [intentional] accident at Fukushima and that death was from falling debris.

How many coal miners were killed in that accident a couple of years ago? Where was that again?
