Dumbass Dave has a point.

Insurance companies DO determine "what is covered and what is not."

All of this is spelled out up front, in writing, in the insurance contract. Medicare is exactly the same.

Any intelligent patient can read the contract and understand what is covered. Claims are generally turned down only because they are not covered by the policy.
