Go to the state license website

Use the drop down menu to click on physician.
Input Horn first initial G.
There are two. One is your person. Click on the links. You will find the official records concerning his license.

Next go to the Cook County Clerk of Circuit Court website.

On teh left, click on on line case information. It will give you a drop down menu. Click on Electronic Full Case Docket search.

For division, click down to Law Division

Enter Horn Gerald. Click defendant box. You will get a list of the cases in which he has been a defendant in a Cook County law suit for damages over $50,000 since 1994. It doesn't mean he lost those cases; that is just the list of people who sued him. Try alternates on the docket search to see if there are other cases, especially in the civil division.

These are public records. they are free. What you do with the information is your business.

Good luck

I had Laski done by Dr. Horn in Oak Brook almost 2 years ago. My vision is better than 20/20 at 20/10. No complaints here and very satisfied. My wife also had it done and she has no complaints.
With Lasik though, it is surgery. I went back to work too soon, 3 days after surgery. They say you can go back the next day, but ideally you should take a week off if you drive. My biggest problem if you call it a problem was sensitivity to light. I was wearing sunglasses at my desk to stop the glare. There were some halos with lights at night too.
But after a month everything was good thank god. The only difference now is I cannot read text up close an inch from my face like I could before. I have to hold it a bit away. And transitioning immediately from reading up close to looking far away takes a few seconds of focus but overall 100% satisfied.

BTW, everyone's eyes are different. One person's experience won't be one size fits all for everyone.
Have your eyes evaluated, and understand the risks.
That said, if you are a good candidate, you should do well. And don't try saving money with cheaper procedures or anything. It's your eyes after all.

Dr. Horn did my Lasik in 2000, 11 years ago. I was legally blind at -8.5, could not even read the big "E" on a vision test. I has enjoyed better than 20/20 vision for more than 11 years now. Dr. Horn is one of the best Lasik surgeon in the Midwest.

D Horn performed my LASIK procedure in 2004 and I can see better than ever. I had worn glasses since I was 9 years old. I had a wonderful experience. I got cold feet late in the evening before my surgery and he returned my call and talked with me for about 30 mins. It was a wonderful experience.
