If six years of therapy have not succeeded in preparing this man to travel an hour away I, perhaps not popularly, would suggest that two more years would not likely have met the objective. Perhaps he has come as far as he can and has just developed a dependency on his counselor/therapist. Or, the therapist should have focused, in the past year, on his travel issues.
A psychological session costs about $120 to $150 but let's say hypothetically, and generously, that he was seen at a discount rate of $200 month ( 2 sessions) for a six year period. That translates into $2400 a year. Multiply that by 6 years and it cost $14,400.
This is above and beyond taxi rides of two hours and any meal allowance provided above and beyond if applicable.These expenses are probably not far off of the cost of the treatment.
I would not be surprised if we have spent $20,000 already if not closer to $30,000.
I don't believe that all healthcare can be whittled down due to cost but there is a point where we have to take other things into consideration. Is his "large" expense resulting in another vet being refused something because of budget constraints. What is reasonable, do the cab records provide a departure time and a return time. Can he be reached at home a couple of hours after his appointment time. If he is going straight to counseling and straight home it is clear that it is the therapy he is consumed by. If he visits friends, goes to the casino or other things he maybe enduring the therapy to allow himself an outing.
Don't be mean and don't be suspicious? There has to be accountability.

Some of the VA's farm this work out and I am sure this could be farmed out at a lower cost locally. That is why the VA farms this work out because of the cheaper price. Even if it is not at a cheaper price, the cost of this man's treatment should continue because of his medical condition but is it doing him any good? In his years of treatment, is his condition any better? I know someone who has a panic disorder (not a vet) and this person has been to counseling, takes medications and still has issues, so this condition is not easily treatable. It takes two people to help, the patient and the doctor along with any family members who understand this condition. People with this disorder have good days and bad ones but this condition requires continued treatment to deal with the day to day situations that happen. This person I know, that has this condition, was chastised on line for support she gave to another person with bi-polar and the results were disastrous. The person with panic disorder refuses to go on line anymore because of the comments made by this bi-polar person.
These are serious issues that must be addressed and treated and with that in mind, an answer can be found for treatment locally for the best interests of the patient!
