1976 T140 mounting upper and lower yoke questions...

I am having a hard time understanding the replacement parts catalogue for the required stem parts. It indicates 2 abutment rings (what is an abutment ring and where do they go), two roller bearings (I got that) and two Dust covers which apparently go under the bearing cones?
The problem I'm having is that the Steering stem nut doesn't go all the way down to the top yoke when trying to tighten it. It apparently is tighting down on top of the top bearing (which appears not to be original.
Another question I have is regarding the Steering Stem Nut. Is it suppose to go through the center of the top bearing or is it supposed to be tight before it touches the top of the bearing? Any help would be appreciated -
