I have a 2000 Camry that runs rough, idles too low at a stop when in gear. It is fine in neutral but sounds like a sewing machine when in gear. It gets around 27 mpg highway.

Improper distribution of current to the plugs causes rough idle and very high gas consumption. When was the last time you changed your distributor cap?
"The distributor cap for the Toyota distributes electricity from the ignition coils to the spark plug wires in the right timing. It is normally built with hard plastic and a small amount of copper."

Make sure your spark plugs are ok and gapped correctly.
Make sure your air filter is clean.
You could also clean the throttle body as it gunks up due to oil vapour from the PCV system.
Also check the inside of the distributor cap as it can have some gunk on the pin surface which you can scrape away if not too bad or replace the cap if rather worn.
Then set the base idle speed to 700RPM (man or 750RPM (auto).
Also check the idle speed control systems (turn of air-con & connect TE1 and E1 of the check connector and the idle speed should increase to 1000-1200RPM before returning to 800RPM (man) 850RPM (auto)

Sounds like a stuck EGR valve. Have it checked by a shop. If that's what it is, it's not a big deal to fix.
