NTSB: Flight instructor's failure to abort caused crash that killed three

The flight instructor's failure to abort the flight has been found the probable cause of a crash that killed the three people aboard a small airplane after the pilot and instructor failed to extend the wing flaps correctly on takeoff at Mohawk Valley Airport in Scotia, according to the National Transportation Safety Board.
