SAEGERTOWN, Pa. - Federal and state officials are investigating the workplace death of an Ohio man who's skull was crushed in a well-drilling accident in northwestern Pennsylvania.

James Maxton Johnson passed away peacefully at his home surrounded by caring friends on Tuesday, June 9, 2009 after a lengthy illness.
